Details of Experience:

Ms. Amolsch came to Manchester Center after being referred from St. Thomas Rutherford, where she was admitted for complications from COPD, malnutrition, and recurrent falls. Ms. Amolsch was reluctant, at first, about coming to a skilled facility for care.

Upon arrival, she was quickly met by many of our team members…our Nurses, Social Services, Concierge, and our Therapy team. Once evaluated by the therapy team, they concluded she could only walk around 40 ft and needed assistance with her activities of daily living (ADLs).

She made great progress during her time here. Over six weeks, she worked hard with our therapy team to regain her strength, balance, as well as endurance. During her stay, her nutrition also improved.

She was very grateful for the care she received. She stated, “they have given me the greatest gift…to be able to walk again. I can’t thank Manchester Center enough!”

Details of Experience:

Mr. Matthew came to Manchester Center after a brief stay at Vanderbilt Tullahoma Horton Hospital, where he was admitted for issues involving his Parkinson’s disease, diabetes, and COPD. He also had issues with altered mental status. He was then referred over to Manchester Center!

Upon arrival, he was quickly greeted by many Manchester Centers team members, such as Social Work, Activities Director, our therapy team, and many more. After he settled in, he was evaluated by the therapy team. At that time, it was noted that Mr. Matthews required Mod/Max assistance for his daily living activities (ADLs) and could only walk a few steps. His family was considering long-term care for Mr. Matthews. He eagerly worked hard on his therapy over a few months. He reached the point where he could complete all ADLs independently and use a rollator to walk household distances.

The patient was discharged home with his wife. Mr. Matthews, you accomplished while here at Manchester Center.

Details of Experience:
Ms. Phillips first arrived at our community on 07/23/21 with pneumonia, causing COPD and respiratory failure complications, making the patient weak, fatigued, and oxygen-dependent.

Upon arrival, she was met by several of our team members, such as Social Services, Recreational Therapy Dietary, and many more. She felt very welcomed. Ms. Phillips was a very upbeat lady with the drive to get better. When our therapy team initially evaluated Ms. Phillips, she could only walk about 50 feet and was easily fatigued and oxygen-dependent when ambulating. She was a high fall risk during her activities of daily living (ADLs). After working for three weeks with our wonderful therapy team, she was able to transition back home safely and complete all ADLs independently…walking 300 feet before becoming fatigued.

During her stay, our Concierge spent some time chatting with her. She told us she was once a CNA. She understood the work, and she was so happy with the nursing staff in our community. Everyone made her feel welcome, and she had a wonderful stay.

Details of Experience:
Mr. Phillip Barnes was admitted into Manchester Center for Rehabilitation and Healing after being hospitalized for shortness of breath due to bilateral pneumonia, pleurisy, and left pleural effusion.  

Upon arrival at Manchester Center, Mr. Barnes was met by many of our team members: Rehabilitation Services, Social Services, Concierge, and many more. He was eager to begin his therapy, so he could return home to his wife and continue to work in his yard. 

When Mr. Barnes first came to  Manchester Center, he was weak, and his balance was off. He was oxygen-dependent and required contact guard assist (CGA) for bed mobility. He did transfer with minimal assistance. Mr. Barnes also had issues with climbing stairs. 

Within weeks of being at Manchester Center, his posture had improved, and he was independent with bed mobility and no longer dependent on oxygen. Mr. Barnes was happy with the care he received at Manchester Center. He returned home with his wife and continued therapy with home health.

Details of Experience:
Mrs. Morris arrived after being hospitalized at Unity Medical Center for issues relating to AFib and CHF. Upon arrival, Mrs. Morris was not pleased about being at a rehab center.  She didn’t feel she needed rehab and wanted to go home. 

She was quickly met by our many team members here at Manchester Center,  Recreation, therapy,  social worker, Concierge, and several more. She decided to stay and give it a try. When first admitted at Manchester Center, Mrs. Morris had difficulty walking due to weakness and was a significant fall risk. She required extensive assistance with her activities of daily living (ADLs). 

After spending about three weeks with us and working really hard on her therapies, she was discharged home. She is now able to walk up to 200 ft with her walker and can complete all her ADLs by herself. 

When the Concierge asked her about her stay, she stated, “everyone helped her to improve, and she was happy she decided to stay and get the help she needed!

Details of Experience:
On admission, the patient had orthostatic hypotension issues, which made it hard for him to walk. He was unable to take steps or even stand with assistance. 

After being at Manchester Center for four weeks, he was discharged back to his Assisted Living Community and is now able to walk up to 200 feet with his walker, unassisted, and is meeting all his ADLs without assistance.

Ms. Janes was admitted to Manchester Center on March 19th from StoneCrest Medical Center, Smyrna, Tennessee. Ms. Janes was greeted by many of our team members: Nursing, Concierge, Rehabilitation Services, and many of our Administrative team. 

When our therapy team evaluated her, it was noticed that she needed significant assistance with all ADLS (Activities of daily living). She was walking less than 100 ft at a time. Ms. Janes worked with Physical and occupational therapists for about a month.

Details of Experience:
Mr. Thacker arrived at Manchester Center for Rehabilitation and Healing on January, 30th 2021.
After a short stay at St. Thomas West Nashville, where he was under evaluation and the management of ventricular tachycardia. It was quickly noted that he was in need of physical and occupational therapy to help with muscle weakness and (ADLS) activities of daily living.

He was then referred to Manchester Center for Rehabilitation and Healing for his rehabilitation needs. Once Mr. Thacker arrived at Manchester Center, he was greeted by many of our professional team members. Within the first 24 hours, he was visited by Physical Therapy, Concierge Services, and many other Administrative team members.

When first admitted to Manchester Center, Mr. Thacker could walk less than 50 ft. with a rolling walker and needed significant assistance with bathing and toileting due to weakness. Within two weeks of being with us, he was walking 100 ft and transitioning to a cane to ambulate. He was also able to complete his ADLs independently.

Mr. Thacker was discharged safely to an assisted living community in the area.

Details of Experience:
Mr. Fults arrived at Manchester Center for Rehabilitation and Healing on December 31st from St. Thomas Midtown in Nashville, TN. Mr. Fults was diagnosed with COVID–19 on December 11th and admitted to St. Thomas Midtown for complications from COVID –19. Mr. Fults also was experiencing generalized weakness.

Doctors soon decided he needed rehabilitation, and Mr. Fults chose to go to Manchester Center. After Mr. Fults arrived at Manchester Center, he was greeted by multiple staff members. Within the first 24 hours, he was greeted by the Therapist, the Concierge, and many of the administrative team who then answered many of his questions about his care while here. Mr. Fults started off slowly with therapy. He continued to show a functional decline even after his recovery from Covid –19. He presented with low endurance, only able to walk about 25 feet before becoming fatigued. He also required assistance with toileting and dressing, which in the past he had done himself. Mr. Fults tried his hardest even while maintaining his dialysis appointments.

Within a few weeks of therapy with our team, Mr. Fults began to walk further and regain his strength.
Upon discharge from Manchester Center, Mr. Fults was able to walk 50-100 feet and take care of his activities of daily living (ADLs) independently. Mr. Fults was able to return home with assistance from home health.

How did this patient hear about Manchester Center for Rehabilitation and Healing? Mr. Hipps had been to our community in the past for Rehabilitation Therapy after his Heart Surgery.


Details of Experience:
Mr. J. Hipps lives with his wife in Altamont, Tennessee. He went to Unity Medical Center on December 9th, 2020 following a fall caused by his vertigo. Emergency room physicians suggested that he should receive skilled nursing and rehabilitation therapy, and his first choice was to come back to Manchester Center for his rehabilitation.

Once he had arrived at Manchester Center, Mr. Hipps was greeted by many of the knowledgeable, kind team members to discuss his needs and the goals he would like to meet. Within his first few days, he was working with the rehabilitation department. Therapy quickly identified his issue as Benign Paroxysmal Positional Vertigo (BPPV), which is a common type of vertigo. They then provided appropriate vestibular treatment and exercises to improve the patient’s issues.

When Mr. Hipps first arrived, he couldn’t sit on the side of the bed without an onset of dizziness. This made him afraid to attempt to ambulate. After successful treatments and exercises, he was able to discharge back home with his wife, using a walker to ambulate up to 100 ft.
